None of us want to think about it, but at some point in the future, we all may face a situation where important decisions must be made about our healthcare, but we are unable to speak for ourselves. Or perhaps this will happen to a loved one. Is your family prepared? Come to this workshop to learn how to get your ducks in a row. This will be an informative workshop about advanced care planning, healthcare, proxies, legal paperwork, helpful handouts like the Five Wishes living will form, the NC Practical Form, and an interactive exercise that will clarify your thoughts surrounding this process.Our presenter will be Betsy Barton, MPH, CT and the Director of Special Projects at Transitions LifeCare.Registration requested.
